At The Fourth, we believe the best time to plan your postnatal care is before you need it, but if you’re already there, it is never too late to prioritise getting the support you personally need.

We exist because the fourth trimester deserves the same level of attention and preparation as pregnancy and birth itself. Our programmes are built around the realities of recovery: every element is tailored to you, your mind, your body, and your recovery.

You do not have to be struggling to deserve care. You simply have to be a mother. And the earlier you begin planning, the more we can do for you.

Now. The single most important thing we can tell you is that the best time to plan your fourth trimester is before your baby arrives. Most mothers leave postnatal planning until after birth - when they are exhausted, overwhelmed, and least equipped to make decisions. The women who transition most smoothly are those who have their village in place before they walk through the door.
We recommend booking your free consultation from around 20 weeks onwards. This gives us time to understand your needs, introduce you to your care providers, and ensure everything is ready the moment it matters.

The fourth trimester is the 12 weeks following birth - a period of profound hormonal, neurological, and physical change. Oestrogen and progesterone drop dramatically after birth. Cortisol rises. The nervous system reorganises itself entirely. Sleep is fractured. The body heals from one of its most significant physical events.
The hormonal, physical, and psychological changes of becoming a mother can continue for up to a year.
In the UK, structured postnatal support for mothers is almost entirely absent. The Fourth exists to change that. Planning for your fourth trimester is not a luxury - it is an investment in your recovery, your mental health, and your experience of early motherhood.

It should consider: who will be with you in the first days, what practical support you will have at home, how you will be nourished, what physical recovery treatments will support your body, what emotional support you have access to, and what happens when your partner returns to work.

Antenatal classes are brilliant at providing information on pregnancy and what to expect around birth. Birth doulas can be incredible - advocating for you, supporting you through labour, and being a reassuring presence in the birth room. We have enormous respect for both.
Where The Fourth goes further is in what comes next. Once your baby arrives, the structure and support often disappears, just at the moment you need it most. The Fourth coordinates a complete, multi-service programme across all trimesters, with a special focus on the fourth - so you are not finding, vetting, and managing multiple providers yourself in the most exhausted months of your life. We do that for you. All of it.

Yes — and we strongly recommend it. Booking during pregnancy allows us to design your programme thoughtfully, introduce you to your care team in advance, and ensure everything is in place from the moment you come home. A 50% deposit secures your booking, with the remaining balance due approximately seven days before your programme begins or your due date. If your baby arrives early, we adapt around you.

It is never too late. Even if you are days from your due date, please reach out. We will always do what we can to put support in place quickly. A programme booked late is infinitely better than no programme at all. Contact us at bookings@thefourth.uk and we will move as fast as we can for you.

We offer two mindfully curated programmes to get you started — Home From Hospital, designed for your first weeks at home, and The First 30 Days, designed for your first month of recovery. Both are planned before your due date so that support is in place from the moment you need it.

Absolutely - and please do reach out. Many of the women we support come to us after birth, sometimes days postpartum and sometimes weeks in, when the reality of recovery has set in and the support they hoped for hasn’t materialised.

You are not alone and you are not failing. Postnatal anxiety and depression are profoundly common and profoundly underserved. The hormonal and neurological changes of the fourth trimester create real, physiological conditions - not a reflection of your capabilities or your love for your baby.
The Fourth is not a clinical mental health service, but our programmes are designed to support the conditions in which mental health can improve: rest, nourishment, physical care, practical help, and the relief of not having to manage everything alone. Many of our clients find that the practical and physical support we provide creates the space they need to begin to heal emotionally too. We always recommend working alongside your GP or a perinatal mental health specialist. Please reach out ASAP if we can support you.

This is one of the most common moments we hear from mothers - the point at which the early support falls away and the weight of it lands. We can be there. Whether you need a doula to take care of your baby so you can sleep, meals arriving at your door, a massage to help your body recover, or simply a professional, warm presence in your home - we build around what you need right now. Yes. Our therapists specialise in holistic and lymphatic drainage massage for pregnancy and postnatal recovery, delivered to your home. Holistic massage soothes the mind, boosts circulation, and supports hormonal rebalancing. Lymphatic drainage uses gentle techniques to reduce swelling and support physical healing. Both are designed for the specific needs of a body recovering from birth. 60 and 90 minute treatments are available for expectant and new mothers, partners, and any additional family members who would enjoy a treatment whilst at your home.

Yes. We offer both foot and facial reflexology as part of our programmes, for women during pregnancy and in the postnatal period.
Foot reflexology (60 minutes) works across each body system to create deep relaxation, support physical recovery, and restore energy after birth. Facial reflexology (30 minutes) combines reflexology principles with Gua Sha techniques to deliver a pregnancy and postpartum-safe facial - as restorative as it is beautiful.
All treatments are delivered to your home by reflexologists who specialise in pregnancy and postnatal care, and understand the unique needs of a body growing or recovering.

Yes. Our pre and postnatal nutrition service delivers fresh, organic, gluten-free, home-cooked meals with Ayurvedic roots directly to your door - enough for two adults.

Yes. We hold this question with the greatest care and tenderness.
The physical and hormonal experience of pregnancy and birth does not end with loss. Your body still undergoes profound change - your hormones still shift, your milk may still come in, your body still needs to recover. And your heart carries something words can barely reach.
We work with women who have experienced miscarriage, stillbirth, or neonatal loss who need physical and emotional support during the postnatal period. Our care in these circumstances is led entirely by you - what you need, what pace feels right, what kind of presence helps. We do not make assumptions. We simply show up, with gentleness and without pressure.
